Pre-requisite Validation Form

This form is for students requesting to register for a course with a prerequisite.
- The student must provide an UNOFFICIAL copy of the transcript showing that the prerequisite was successfully completed with a grade of “C” better or the applicable SAT/ACT scores.
- A copy of the transcript or SAT/ACT scores must be sent the Registration and Records Office/Curriculum to be scanned to the student’s record.
For students requesting to receive transfer credit, the student must provide an OFFICIAL copy of the transcript showing that the prerequisite was successfully completed with a grade of “C” or better.
- The official copy of the transcript must be sent to the Admissions Office in a sealed envelope to be documented.
- The transcript will be forwarded to the Admissions Evaluators to be processed for transfer credit.
